Good Faith Estimate:

Under Section 2799B-6 of the Public Health Service Act, health care providers and facilities are required to inform individuals who are not enrolled in a plan, coverage, or Federal health care program, or not seeking to file a claim with their plan or coverage, both orally and in writing, of their ability, to receive a “Good Faith Estimate” of expected charges. You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost. If you receive a bill that is at least $400 more than the Good Faith Estimate, you can dispute the bill.

Requesting your Records

Texas law requires that requests for mental health records be made in writing. To obtain your records, please follow the steps below:

Submit a written request via email to gianna@bewellrelationaltherapy.com, with the subject line “REQUEST FOR RECORDS.”

Include the specific records you are requesting, and the name, address, and/or email address of the intended recipient. I will then send you a Release of Information form via the portal system for you to sign that will authorize me to release your records.

I will then send you your records in a timely manner. Please allow up to two weeks as I may not be immediately available.

Complaints

If you have questions about the treatment you received from me, or any behavioral health provider, you can reach out to the professional Behavioral Health Executive Counsel (BHEC) to learn more about the rules and even file a complaint if that is what feels right.

Additionally, you have the ability to reach out to the Office of the Attorney General's Consumer Protection website.  You can reach out to them any time you feel as though a business has not done what they claim they would.
